Payment Information for CE Credit

If you are planning to request CE Credit at the 2010 Midwest Pharmacy Resident’s Conference and you would like to pay the $15.00 fee online prior to attending the conference; you may do so here. Please be sure to print your receipt and bring it with you to the conference. You will be required to provide your online receipt at the registration desk when submitting your CME Registration and CE Credit Request Forms (attached below) before leaving the conference.

 Please Note:

In order to receive CE credit, participants must complete all speaker evaluation forms during the conference.  Participants must attend the session in it’s entirety in order to receive any CE credit for that session.  No partial credit will be awarded.  Statements of Credit will be mailed to participants requesting credit.  If you do not receive your Statement of Credit, please contact Dana Price at (402) 559-4412.  Please see “sample” forms (also available at the conference) below:
