CE Information

Continuing Education Credit will once again be available for purchase at the 2010 Midwest Pharmacy Residents Conference.  In accordance with CE guidelines, in order to offer CE Credit; ALL PRESENTING RESIDENTS/FELLOWS attending the conference MUST complete and sign the following NCCPE_Conflict of Interest disclosure form.  Once completed and electronically “signed” (if you are not able to provide an electronic signature it is acceptable to type your name on the signature line), the document must be saved in last name, first initial_COI format (example:  Doe,J_COI) and emailed to dprice@nebraskamed.com along with an electronic copy of the presenter’s CV saved in the same format (example:  Doe,J_CV).  CV and Conflict of Interest form must be received no later than February 26, 2010.

The Nebraska Council for Continuing Pharmaceutical Education, Inc. (NCCPE) is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education.  The ACPE provider numbers for May 5-7, 2010 will be posted next to each CE session on the final conference presentation schedule located inside the 2010 conference program booklet.  Participants will be awarded one contact hour of continuing education or 0.10 CEUs per hour of completion.

In order to receive CE credit, participants must complete all speaker evaluation forms and attend each session in it’s entirety in order to receive any CE credit for that session.  No partial credit will be awarded.  Statements of Credit will be mailed to participants requesting credit.  If you do not receive your Statement of Credit, please contact Dana Price at (402) 559-4412.  Please see “sample” forms (also available at the conference) below:
