Presenter Checklist

 

 

Residents and Fellows who wish to attend the conference must register for the conference by completing an online registration form.  Attendees will also be required to pay a registration fee.  Click here to register for the conference:  Online Registration.  Please see below fee stipulations:

*  Residents/Fellows (accompanied by a preceptor) registering by February 26, 2010 =  $260.00 
*  LATE REGISTRATION FEES:  Residents/Fellows (accompanied by a preceptor) registering February 27 – March 04, 2010 =  $315.00 

*  Residents/Fellows (unaccompanied by a preceptor) registering by February 26, 2010 = $315.00
*  LATE REGISTRATION FEES:  Residents/Fellows (unaccompanied by a preceptor) registering February 27 – March 04, 2010 =  $370.00

*  NO REGISTRATIONS WILL BE ACCEPTED AFTER MARCH 04, 2010

 

  • HOTEL RESERVATIONS:

Presenting Residents & Fellows (accompanied by a preceptor):
If you are a presenting resident or fellow attending The Midwest Pharmacy Residents Conference with a preceptor; please be sure to complete an online registration form in it’s entirety no later than February 26, 2010.  (Registrations received after February 26 will be subject to a $55.00 late fee).  No Registrations will be accepted after March 4, 2010.  If you plan to share a room with a preceptor; please make note of these arrangements in the “Special Request” section of the online conference registration form. 

Registering for the conference will also ensure that presenters attending from outside the Omaha/Metropolitan Area are placed on The Embassy Suites master rooming list within the conference room block.  The Conference will pay for room + tax for up to two (2) nights stay for presenters  (accompanied by a preceptor) attending the conference from outside the Omaha/Metropolitan Area.

Presenting Residents & Fellows (unaccompanied by a preceptor):
If you are a presenting resident or fellow attending The Midwest Pharmacy Residents Conference without a preceptor; please note that the conference is not responsible for paying your hotel accomadations.   If contacted; the conference coordinator may (under certain circumstances) decide to request that the conference consider paying for the unaccompanied presenter’s room + tax for up to two (2) nights stay; however approval is not guaranteed.  If the unaccompanied presenter is granted a “conference-paid” room; he or she would be required to share that room with another attending resident/fellow who has requested a room but did not specify a roommate.

The unaccompanied presenter must also complete an online registration form in it’s entirety no later than February 26, 2010.  (Registrations received after February 26 will be subject to a $55.00 late fee).  No Registrations will be accepted after March 4, 2010.  Registering for the conference will also ensure that unaccompanied presenters attending from outside the Omaha/Metropolitan Area are placed on The Embassy Suites master rooming list within the conference room block.

 

  • ABSTRACT SUBMISSION:

Verify that your abstract is properly prepared and submitted (SeeAbstractGuidelines):

*  Abstract prepared according to “Abstract Guidelines” and/or “Sample Abstract” on our website under “Call for Abstracts”?
*  Abstract saved in the following format?  (example:  Doe,J_abstract)
*  Abstract emailed to dprice@nebraskamed.com ?

 

In order for the Midwest Pharmacy Residents Conference to offer it’s attendees continuing education credit;  ALL PRESENTERS must submit a CV along with a complete and electronically signed NCCPE_Conflict of Interest disclosure form in accordance to CE Guidelines:

*  CV saved in the following format? (example:  Doe,J_CV)
*  CV emailed to dprice@nebraskamed.com ?
*  Conflict of Interest Disclosure form complete and electronically signed?
*  Conflict of Interest Disclosure form saved in the following format?  (example:  Doe,J_COI)
*  Conflict of Interest Disclosure form emailed to dprice@nebraskamed.com
