Presenter Checklist
Residents and Fellows who wish to attend the conference must register for the conference by completing an online registration form. Attendees are required to pay a registration fee. Click here to register for the conference: Online Registration. Please see below fee stipulations:
Registration for (Resident/Fellow) Attendees who DO NOT REQUIRE HOTEL ACCOMMODATIONS:
* Accompanied by a preceptor – Must register by February 24, 2012 = $260.00
* LATE REGISTRATION FEES: Accompanied by a preceptor registering February 25 – March 05, 2012 = $315.00
* Unaccompanied by a preceptor – Must register by February 24, 2012 = $315.00
* LATE REGISTRATION FEES: Unaccompanied by a preceptor registering February 25 – March 05, 2012 = $370.00

Registration for (Resident/Fellow) Attendees who REQUIRE HOTEL ACCOMMODATIONS (includes up to 2-nights stay for Residents/Fellows from OUTSIDE the Omaha/Metro Area Only - Rooming Fee Applies)
* Accompanied by a preceptor – Must register by February 24, 2012 = $310.00
* LATE REGISTRATION FEES: Accompanied by a preceptor registering February 25 – March 05, 2012 = $365.00
* Unaccompanied by a preceptor – Must register by February 24, 2012 = $365.00
* LATE REGISTRATION FEES: Unaccompanied by a preceptor registering February 25 – March 05, 2012 = $420.00

****Hotel reservations for presenters (accompanied by a preceptor) traveling from OUTSIDE the Omaha/Metropolitan Area ONLY will be made by The Conference based on information provided during online registration. The Conference will assess a $50.00 rooming fee during conference registration for all residents/fellows requiring hotel accommodations. Room + tax (up to two nights only) will be paid for by The Conference. If you are a presenting resident or fellow from OUTSIDE the Omaha/Metro area (accompanied by a preceptor) and would like to have a room reserved at the Embassy Suites Hotel (conference host hotel) please complete the conference registration form in it’s entirety including your preferred roommate name(s) in the space provided and the dates that you will be arriving and departing the conference. If you are a presenter unaccompanied by a preceptor please refer to the “LODGING & TRAVEL” tab above for additional information. If a preceptor is planning to share a room with a resident, the conference must be notified – please also make note of this request in the “Special Instructions/Requests” section within the conference registration form. Presenters who request a private room will be charged for half of the cost of the room. Private room requests will be accepted space permiting.****
Presenting Residents/Fellows (accompanied by a preceptor) from OUTSIDE the Omaha/Metro Area – REQUIRING HOTEL ACCOMMDATIONS: If you are a presenting resident or fellow attending The Midwest Pharmacy Residents Conference with a preceptor; please be sure to complete an online registration form in it’s entirety no later than February 24, 2012. (Registrations received after February 24 will be subject to a $55.00 late fee). No Registrations will be accepted after March 5, 2012. If you plan to share a room with a preceptor; please make note of these arrangements in the “Special Request” section of the online conference registration form.
Registering for the conference will also ensure that presenters attending from outside the Omaha/Metropolitan Area are placed on The Embassy Suites master rooming list within the conference room block. A $50.00 rooming fee will be assessed by The Conference during online conference registration. The Conference will pay for room + tax for up to two (2) nights stay for presenters (accompanied by a preceptor) attending the conference from outside the Omaha/Metropolitan Area.
Presenting Residents & Fellows (unaccompanied by a preceptor):
If you are a presenting resident or fellow attending The Midwest Pharmacy Residents Conference without a preceptor; please note that the conference is not responsible for paying your hotel accommadations. If contacted; the conference coordinator may (under certain circumstances) decide to request that the conference consider paying for the unaccompanied presenter’s room + tax for up to two (2) nights stay; however approval is not guaranteed. If the unaccompanied presenter is granted a “conference-paid” room; he or she would be required to share that room with another attending resident/fellow who has requested a room but did not specify a roommate.
The unaccompanied presenter must also complete an online registration form in it’s entirety no later than February 24, 2012. (Registrations received after February 24 will be subject to a $55.00 late fee). No Registrations will be accepted after March 5, 2012. Verify that your abstract is properly prepared and submitted (SeeAbstractGuidelines):
* Abstract prepared according to “Abstract Guidelines” and/or “Sample Abstract” on our website under “Call for Abstracts”
* Abstract saved in the following format: (example: Doe,J_abstract)
* Abstract emailed to dprice@nebraskamed.com

CE REQUIREMENTS:
In order for the Midwest Pharmacy Residents Conference to offer it’s attendees continuing education credit; ALL PRESENTERS must submit a CV along with a complete and electronically signed NCCPE_Conflict of Interest disclosure form in accordance to CE Guidelines:
* CV saved in the following format: (example: Doe,J_CV)
* CV emailed to dprice@nebraskamed.com
* Conflict of Interest Disclosure form complete and electronically signed
* Conflict of Interest Disclosure form saved in the following format: (example: Doe,J_COI)
